Today I was asked to take home a self-evaluation, (without pay) and fill it out for “just 20 minutes”. I politely informed him that I did not want to cause trouble but I did not want to take any work home. I was then told that time would be made for me to do it at work. This was the end of it. Upon relaying this to a couple of my (non-co-worker) friends they told me I was legally required to report him. Is this true? I do not want to.

It sounds like you handled this perfectly. Just as we teach in our Respectful Workplace class, you raised your concerns respectfully, and received what you were entitled to. You do not have to take this any further.
